Tax Clearance Information Form
Due to your status as a US resident a withholding tax of $2,257.00 (USD) has been imposed on your winnings. North American Lottery Corporation is required to report all foreign payouts and this tax must be paid by the prize winner prior to the release of your prize.
Unfortunately we cannot pay the said tax on your behalf without first obtaining Power of Attorney. This slow process may subsequently result in the expiration of your claim. For that reason, we have taken the time and money to forward this information as quickly, confidentially and safely into your possession. These are legal bonding documents and this is not a solicitation.
After fully reviewing documentation, you will be expected to make your tax payment as instructed by your Claims Advisor. Upon payment of the tax, full total of category "D" prize winnings in the amount of 89,018.00 (USD) will be forwarded to you via a Cashiers Check to your home address or Wire Transfer into your personal bank account. It is solely your responsibility to pay and taxes your government requires. Our company does not forward information to the Internal Revenue Service (IRS) however; there are severe penalties for income tax evasion including large fines, and/or imprisonment.
IMPORTANT NOTICE
Do not attempt to secure tax payment without first contacting our office. Due to incidents of mail theft we have instructed our bank and administration team not to honor any payments which has not been verified by our Claims Office.
In order to avoid double claims and unwarranted abuse of the program you are expected to keep all information pertaining to your prize confidential until your claim has been processed, and your funds dispersed.
